They synapse via glial cells in the spinal cord’s lamina II, IV, V, and VI. The pain signal is sent to the brain via the spinothalamic tract to the thalamus, where the limbic system processes and stores information.

The type IV fibers, the initiators of the synaptic activity that leads to pain perception, also have a story of their own. When activated, they release Calcitonin gene-related peptide (CGRP) and substance P, both of which promote an inflammation response. This response triggers immune mediating cells like macrophages, mast cells, and endothelial cells to protect the body. However, when these type IV fibers are overactivated, these inflammatory mediators are called into action too frequently, leading to excessive inflammation throughout the body.
